Abstract Introduction/Objective Langerhans’ cell histiocytosis (LCH) involving the lungs usually presents as multiple interstitial, bilateral, peribronchiolar pulmonary nodules. As are a common site of metastatic involvement LHC can masquerade lesion. The distinction be challenging, especially on frozen section. Methods/Case Report We present case 35-year-old male with history melanoma who presented nodules and underwent lung wedge resection. Frozen section was requested at time surgery. Grossly there 2.8 cm tan-grey irregular lesion abutting pleura. representative piece showed discohesive composed large cells prominent nuclei resembling melanocytes. eosinophilic infiltrate in slide extremely scant, presence anthracotic macrophages resembled melanin pigment frequently seen melanoma. CT imaging prior to surgery had raised differential LCH disease. favored negative for carcinoma final diagnosis deferred permanent sections. Permanent tissue sections classic features positivity CD1a confirming diagnosis.
